The fight against HIV in the United States is at a critical point. The recent approval of lenacapavir, a revolutionary drug for HIV prevention, is overshadowed by significant funding cuts to US public health agencies and global aid programs. Experts are concerned that these cuts could undermine efforts to end the HIV epidemic in the US by 2030.

Lenacapavir: A Breakthrough at Risk?

Lenacapavir, hailed by UNAIDS as a "miracle product" and recognized by Science as a breakthrough, offers six months of protection against HIV infection with a twice-yearly injection. Mitchell Warren, executive director of AVAC, questions whether this unprecedented opportunity will be squandered due to defunding.

Why Lenacapavir is Different

Unlike other PrEP drugs, lenacapavir attacks HIV at multiple stages of its lifecycle, making it highly effective. While around 400,000 US patients currently use daily PrEP pills, lenacapavir provides longer-lasting protection with just two injections per year.

The Impact of Funding Cuts

Despite previous efforts to combat the HIV epidemic, current budget cuts are affecting vital agencies like the NIH and CDC, along with programs like Medicare and Medicaid. Global initiatives, including USAID and PEPFAR, have also been reduced. These cuts disproportionately affect vulnerable populations, potentially denying them access to lenacapavir.

Jeffrey Crowley from the O'Neill Institute emphasizes the positive impact of expanded Medicaid coverage on access to HIV programs. He warns that the funding reductions jeopardize these gains and risk reversing years of progress in the fight against HIV.
